The template looks fine. I just made one small change. We have tried to standardize the displayed text for the scanner link to be the RISM sigla for that particular library. You can search for the sigla here.

how you determine whether a work is in PD or not if the compose's dates are unknown Research, but there are many cases in which it has proven impossible to determine when a person has died (the determining factor for two of the three copyright jurisdictions that affects IMSLP--Canada and the E.U.)....

Try re-uploading your arrangement but change the file's name before you do so as it appears that you were using the same file name as that of Manuel Pancorbo's arrangement. The system detected the file you were uploading as a duplicate of that existing file.

Using the Cui template is probably the best way to do it. If you want to change the background color to something different you can find a list of web safe colors with their decimal equivalents here: web colors.

It appears that the problem was due to a composer page existing for you that was improperly named: your first and last names were switched. I've moved your composition to the correct category (Frykman, Gervais) and have associated your username with that category. See if you can upload scores now.

I don't think isozvaillancourt means that the works are actually from the Library of Congress but was using that as an example. A template could be created that could go in either the scanner field or in the miscellaneous notes. Part of the function of the template would be to include the work page ...
