Haven't really found a way explaining how to do it.

A large number of libraries have existing templates that can be used in the scanner/scanned by field. The list can be found here. So, if a score originated from the Staatsbibliothek zu Berlin you can simply input {{SBB}} in the scanner field to link to the library. Additionally, many of the templates have additional variables that will allow you to link to the score's page on the library's website.
